INSTRUCTIONS FOR MINORS TO COMPLETE THE APPLICATION FOR A WORK PERMIT:

SECTION A: Please print the required information. DO NOT COMPLETE THE “EVIDENCE OF AGE” AT THIS TIME. THIS WILL BE COMPLETED LATER.

SECTION B: DO NOT COMPLETE AT THIS TIME. This section must be completed in the presence of the issuing officer.

SECTION C: Please take the application to your prospective employer. Please have your employer complete all of Section C.

SECTION D: Please take the application to a physician. The physician must choose one of the statements and sign the application.

After completing the above steps please take the application to the receptionist in the Tredyffrin/Easttown Administration Offices at 940 West Valley Road Suite 1700 Wayne, PA 19087.

EVIDENCE OF AGE WILL BE REQUIRED AT THAT TIME. SECTION A LISTS THE ACCEPTABLE DOCUMENTS. A PARENT, GUARDIAN, OR LEGAL CUSTODIAN MUST APPEAR IN PERSON TO SIGN SECTION B.
